This form is to be used if you need a new debit or ATM card. If your card was lost or stolen, please call 319-378-0101 (after hours, call 1-800- 554-8969) to prevent possible fraud or identity theft. In the case of theft, be sure to report it to the local authorities, as well.

If you have never had a debit or ATM card with Linn Area Credit Union, please call us at 378-0101, x0 before filling out this form.

You will be charged a $5 fee (plus tax) for each card replaced. If the joint owner of the account also needs a new card, please complete the appropriate fields.
